The Scourge and the Plague of Undeath Explained: Lich King, Cult and Plaguelands
The Plague of Undeath is the magical affliction the Lich King unleashed on Lordaeron, and the Scourge is the undead army it creates, which in the original game holds the Plaguelands, Stratholme and the Naxxramas stronghold. In World of Warcraft: Forever (WoW Forever), set in the first year of the original game, Warcraft Wiki lists the Ruins of Lordaeron boss The Baron as Scourge-affiliated.
Quick answer: The Lich King, the spirit of the orc shaman Ner'zhul, serves the Burning Legion. He had Kel'Thuzad and the Cult of the Damned spread the plague through grain; victims die and rise as the Scourge. By the original game's era, Warcraft Wiki says, the Scourge in Lordaeron was concentrated in the Plaguelands.

A sickness in the grain became an army. This is how it travelled from Northrend to Andorhal and the towns around it.

What is the difference between the Plague of Undeath and the Scourge?
One is a disease and the other is what the disease builds. Warcraft Wiki's Plague page describes the Plague of Undeath as a magical affliction used to create the Scourge, and its Scourge page calls the Scourge a vast army of the undead serving the Lich King. The Scourge page adds that Kel'Thuzad named the Lich King's growing army the Scourge. Read The Third War Explained for the war it started.
Who spread the plague, and how?
The Lich King needed hands in Lordaeron. Warcraft Wiki says Ner'zhul spread the disease in Northrend's human settlements by will alone, and that Kel'Thuzad was tasked with bringing it south, forming the Cult of the Damned to help. Engadget's "Know Your Lore: The Plagued Dragonflight" says Kel'Thuzad founded the cult in Lordaeron promising eternal life, and that its followers distributed plague cauldrons. Warcraft Wiki says Kel'Thuzad and Mal'Ganis used the agricultural hub of Andorhal to ship infected grain to towns including Brill and Stratholme, and Blizzard Watch says Andorhal's granaries fed half the northern towns. Warcraft Wiki and Engadget agree that those who died rose as zombies serving the Lich King. Read The Story of Kel'Thuzad.
Who leads the Scourge?
The master of the Scourge is a former orc. Warcraft Wiki says the Scourge served the Lich King, Ner'zhul, who was under Burning Legion control, and Engadget describes him as a spirit formed from a former orc leader serving the Legion. Both say he commanded his forces from the Frozen Throne. Arthas Menethil later became the Lich King's champion, and Warcraft Wiki says he took Frostmourne and returned to Lordaeron with a Scourge army. Read The Story of Arthas Menethil.
Where is the Scourge in the original game?
Warcraft Wiki says that by 25 years after the Dark Portal, the original game's era, the Scourge in Lordaeron was primarily concentrated in the Plaguelands, with large concentrations in Andorhal and Scholomance, and that its main base was Stratholme under Baron Rivendare. Engadget says Scholomance lies beneath Caer Darrow in the Western Plaguelands and was founded by Kel'Thuzad, and that plague cauldrons are scattered across the Eastern and Western Plaguelands. Warcraft Wiki's Plague page adds that cauldrons spread the plague into the air and the Argent Dawn found ways to sabotage them. Read The Argent Dawn Explained and Western Plaguelands.
